LAPAROSCOPIC REPAIR VERSUS OPEN SURGERY FOR VENTRAL HERNIA: A MULTICENTRIC COMPARISON OF PATIENT OUTCOMES

Abstract

BACKGROUND: Ventral hernias are common, often managed through open surgery, which is associated with higher complications.

OBJECTIVES: The comparison of pain intensity, quality of life, post-operative mobilization, complications between open and laparoscopic ventral hernia repair methods.

METHODOLGY: This multicenter prospective observational study was conducted across four medical institutions in Pakistan over 18 months (March 2023–September 2024). A total of 140 patients were enrolled: 45 from Peshawar Institute of Medical Sciences, 40 from Northwest General Hospital & Research Center, 32 from Gomal Medical College, and 23 from Bannu Medical College. Patients were equally divided into two groups, with 70 undergoing laparoscopic surgery and 70 undergoing open surgery, allowing for an effective comparative analysis.

RESULTS: Laparoscopic surgery significantly improved the quality of life (mean score of 37.61 ± 1.39 at 30 days vs. 29.03 ± 1.97 in the open group) and reduced pain intensity Visual Analogue Scale score of 2.92 ± 0.08 vs. 4.88 ± 0.12, respectively). Mobilization occurred faster in the laparoscopic group (mean 2.04 ± 0.96 days vs. 3.51 ± 0.49). Fewer complications, including infections and recurrences, were observed in the laparoscopic group.

CONCLUSION: Laparoscopic repair offers significant advantages over open surgery in terms of pain reduction, quality of life, quicker mobilization, and fewer complications, making it a superior choice for appropriate patients.
